Why do I get swollen feet and hands all the time? ?

It's the worst at morning, it hurts to walk just down the hall to the shower. It happens during the day if I'm walking around more than normal. It happens at night when I'm laying in bed watching TV. Sometimes I even get the congested head feeling too.. This started about 5 months ago and I'm only 24. I'm kinda worried.
